THE CUSTOMER CERTIFIES THAT (Read carefully before signing)
1. I have authorized TECH-TO-GO to perform the necessary repair listed above.
2. The unit was checked in my presence and the complaint/s so stated are true.
3. I entrusted the accessories listed on this job order.
4. I agree to the terms and condition listed hereunder.
A. Estimate cost to follow. Because defective parts of CUSTOMER is difficult to identify, unit needs
to undergo rigid check up on our workbench using state-of-the-art Electronics Test Instrument before total estimate cost is determined.
B. TECH-TO-GO shall not be answerable for the loss/damage of this unit in the event of robbery,fire,typhoon or other natural calamities.
C. Customer acknowledges that the equipment/data/media may be damaged prior to TECH-TO-GO receipt, and Customer further acknowledges that the efforts of
TECH-TO-GO to complete the repair may result in the destruction of or further damage to the equipment/data/media. TECH-TO-GO for itself regrets that it will not
assume responsibility for additional damages that may occur to the Customer's equipment/data/media during TECH-TO-GO efforts to complete the repair.
D. I agree to pay check up/estimate fee if I discontinue the repair of my unit.
E. In no event will MCSI be liable for any damages whatsoever, including without limitation damages for loss of data, loss business profits,
business interruption, or other pecuniary loss, incidental, consequential, or indirect damages arising from the repair, even if MCSI or any authorized representative has been advised of the
possibility of such damages. Customer acknowledges that the estimated and actual fees and charges reflect this limitation of liability and allocation of risk. the total liability of MCSI or its supplier
to Customer under this Agreement shall in no event exceeed the total sums paid by Customer to MCSI.
F. Customer warrants to MCSI that it is the owner of, and/or has the right to be in possession of all equipment/data/media furnished to MCSI; and Customer will defend, at its expense,
indemnify, and hold MCSI and its suppliers harmless against any damages or expenses that may occur (including reasonable attorney's fees), and pay any cost, damages, or attorney's fees awarded against
MCSI resulting from Customer's breach of This section.
G. Repaired units not claimed within 15 days shall be charged of P 20.00 per day. If not damaged within 45days after notification the repaired units shall be forfeited in favor of MCSI Center to cover the cost of repair and storage.
H. Repaired unit carries a 1 months warranty period. Parts will be shouldered by the customer. Customer will be charge for parts and service if the trouble found is not identical / the same as the problem as previously repaired.
I. The customer must report the loss of claim slip to MCSI Service Center and secure an affidavit of loss immediately. This company shall not help liable for any circumstances arising for the said loss.
J. Customer warrants that all unlicensed software installed in his/her computer are not from MCSI shall not therefore not be liable in any action or suit that may arise for a reason by the installation in the computer of unlicensed software products.
MCSI will not be held responsible in any case units are confiscated due to SOFTWARE COPYRIGHT PROBLEMS.
